New patients should plan to arrive 10 minutes prior to their scheduled appointment time to fill out our health history questionnaire and other necessary paperwork. We will also collect any insurance information you have for us to submit on your behalf. Please bring an updated list of any prescription or over the counter medications you are currently taking to every visit.
